1. Current driver’s license or government issued photo ID.
2. A list of home addresses for the past two years. If renting, include
landlord names, addresses and phone numbers.
3. Current work and home phone numbers.
4. Proof of income for each borrower.
• W-2's (last 2 years)
• Current paystubs (1 month)
• If self-employed: Income taxes, profit and loss, year-to-date
statements and all schedules (2 years)
• If child support or alimony are to be included as income:
A copy of divorce decree/separation agreement and 12 months receipt
• Rental income: Income taxes including Schedule E (2 years)
• Documentation of benefits (i.e. retirement,disability or Social Security)
5. Liabilities
• Creditors' names and addresses (including mortgages)
• Account numbers & current balances on each
• Monthly payment amounts
6. Asset verification
• Checking, savings and investment account statements (2 months)
• Loans, benefits or gift statements
• A copy of the listing contract, purchase agreement, or closing statement, if
a home sale is the source of your cash.
7. The purchase contract signed by all parties (purchases, construction contracts
and lot contracts only).
